How To Request a Note Taker

Students whose documentation supports the need and wishing to have a note taker must do the following: 

1. Read and sign the NOTE TAKER CONTRACT which outlines the student's responsibilities for this accommodation.

Turn this form into Ms. Janelle Cary in Smith Library 410, or Campus Mailbox #5.

2.  Fill out the NOTE TAKER REQUEST form by providing the course/s information.

Email this form directly to Ms. Cary at jcary@highpoint.edu or drop off to her in Smith Library 410, or Campus Mailbox #5.

3. Your request for a note taker will then be followed up on and you will be contacted regarding notes for class.
