Report a Concern
Reports of incidents that allege a violation of the HPU Code of Conduct policies are made through this report. These reports are addressed through the collaboration of the Office of Community Standards and when necessary, Security. Incident Reports must be submitted immediately after the Incident occurs. IRs that are submitted with incomplete, incorrect or inappropriate information will require re-submission by the reporter.

Describe in detail what you witnessed in THIRD (3rd) person and/or the information provided to you by others. Refer to yourself and others by your/their title and last name at all times (Ex. RA Smith, or Security Officer Jones). Provide specific details who was involved, the nature of the incident, the location and time of the incident, as well as language used (i.e. what was communicated, what was observed) by the participants. Only one (1) incident should be reported in each Incident Report Form.

Options To Report A Concern

All members of the HPU Community are able to report concerns to the University.  The different types of reports are listed below.  On-Campus emergencies (medical/safety) should be reported to HPU Security by calling 336-841-9111.  Off-campus emergencies may also be reported to 911. 

Student Complaint: The basis for a student’s complaint may be that an act or situation is objectionable in the view of the student. High Point University provides a process for student complaints to be addressed. Complaints may be about any act, situation, or aspect of life at HPU. Complaints will be addressed by the Office of Community Standards. Students may file a report anonymously but this may limit the University’s ability to thoroughly respond.

HPU CARE Alert: The HPU CARE Team is a multidisciplinary group of professional staff members dedicated to providing support for students in all levels of crisis or who have been identified as exhibiting concerning or worrisome behaviors. The CARE Team takes a holistic approach to identify concerns, assess needs, and implement appropriate interventions.

The main goal of the CARE Team is to help students to be successful through individual support and care and the coordination of HPU resources. Upon receiving a referral, a member of the CARE Team will reach out to the student to discuss the issue(s) they may be facing and to develop a plan to help address any concerns. Students may be connected with or referred to additional supporting offices (The Office of Counseling Services, Peer Assisted Learning Services, Residence Life, Office of Accessibility Resources, Religious Life, etc.)

Please note that the CARE Team should NOT be contacted as first-response to mental health crises/emergencies. If there is an immediate threat or concern for someone’s health or safety, please call High Point University Campus Police & Security at  336-841-9111 or ext. 9111 from a campus phone or call 911.

Care Alert Referrals are processed through the Office of Student Life and not the Office of Counseling Services.

Honor Code Violation:  Reports of academic dishonesty (Cheating, Collusion, Plagiarism) which may violate the University Honor Code are made through this report. These reports are addressed through the collaboration of the reporting faculty member and the Office of Community Standards.

Bias Report: A “Bias Incident” is defined as language or actions that demean, degrade, threaten or harass an individual or group based on their actual or perceived race, color, national or ethnic origin, ancestry, religion, sexual orientation, gender identity, gender expression, disability or status as a military veteran. Bias incidents include intentional and unintentional behavior that includes but is not limited to, name-calling, stereotyping, belittling, inappropriate joking and excluding others based upon their actual or perceived identity. All reports will remain confidential to the extent possible. You may also file a report anonymously but this may limit the University’s ability to thoroughly respond to the reported incident.

Hazing Concern:  Concerns regarding dangerous, illegal, or degrading behaviors as outlined by Fraternity and Sorority Life (FSL), the Student Guide to Campus Life, and North Carolina State law can be reported here.  The University’s ability to address concerns depends on the information provided.  You may file a report anonymously but this may limit the University’s ability to thoroughly respond to the reported incident.

Title IX/Sexual Misconduct Report: Any member of the High Point University community, including visitors and third-party vendors, can use this form to report incidents related to Title IX that occur on or off-campus involving students, faculty, staff and third-party vendors. Title IX includes: Gender Discrimination, Sexual Assault, Sexual Misconduct (viewing/recording, harassment, exploitation), Non-Consensual Sexual Contact, Intimate Partner Violence, and Stalking.
