This form should be used to register any events your chapters plan on doing that will have a large number of off campus guests or will need security or building access’ support in making sure your event goes smoothly.
In efforts to provide a positive experience to all Chapters and their members, The Office of Fraternity and Sorority Life is using this form as a reflection tool for members who decide to leave our organizations. Please know that as students the Office of Fraternity and Sorority Life along with several other services are available for assistance and support.
This form should be used to report any incidents that occurred while serving as an Interfraternity Council Sober Monitor, Lead Fraternity Sober Monitor or Lead Sorority Sober Monitor. Additionally, this form should be used to officially report any incident to the Office of Fraternity and Sorority Life.
The FSL Student Concern form should be submitted immediately but definitely by the end of the semester in which the concern arises. This time frame generally ensures the availability of all parties involved in the complaint. Forms can be submitted anonymously, but provides limited abilities for concerns to be addressed.
This form needs to be completed for any potential new members you are considering adding to your chapter outside of official recruitment processes. Failure to complete this form will result in an automatic IFC Judicial Board referral and significant sanctioning.
