This form should be completed by the Principal Investigator, Lab Manager/Supervisor, or their designee in order to conduct a laboratory hazard assessment specific to the activities performed in their laboratories. The laboratory hazard assessment identifies hazards to individuals and assists in the selection of personal protective equipment (PPE) to protect individuals during work activities. This assessment serves as a step in satisfying the requirements listed in the Occupational Safety and Health Administration (OSHA) standards 29 CFR 1910.1200 (Hazard Communication) and 29 CFR 1910.132 (General Requirements for PPE).
The Laboratory Hazard Assessment should be completed at least annually or upon any new hazard or process introduced to the lab. Completed forms should be submitted to EHS by emailing [email protected]. In laboratory spaces with multiple Principal Investigators, each PI should submit a Laboratory Hazard Assessment form indicative of the work he/she performs.
Upon completion of the Laboratory Hazard Assessment, EHS will provide each PI with a copy of the Hazard Assessment and Recommendations for PPE.
EHS will also install a laboratory safety door placard.
