Use this form to propose:
- Closing an existing program, degree, major, or minor
To discontinue a course, please use the Course Revision Proposal.

- A signed departmental approval page is no longer required.
- The Dean of your school will initiate the curriculum review process once the proposal is submitted. The approval process for these proposals may take up to twelve weeks.
