When to Use This Form

Use this form to propose:

• Revised Learning Outcomes
• All changes related to special topics courses
• Adding or removing an academic program
• Adding or removing a course from the gen ed curriculum
• Changing a course level
• Changing course repeat option
• Changing credits on a course
• Discontinuing a course

Important Updates

• The syllabus in your proposal will be considered a sample. All course content information must be answered on the form.
• A signed departmental approval page is no longer required.
• Proposals should be approved by your Department Chair before online submission. Department Chairs are notified of all submissions and may place proposals on hold if prior approval has not been obtained.
