AD-Hoc Report Request

This request form is for faculty or staff use only.
To request an academically related report, you must fill out the following form and submit it electronically to the Office of the University Registrar. Phone or email requests are not accepted. Please allow a minimum of 5 business days for completion. Requests made during the weeks of registration and/or commencement may require additional time. All requests will be reviewed by the University Registrar before approval. Asterisks indicate required fields.
