Immunizations

All HPU undergraduate and graduate students are required to submit an immunization form in accordance with North Carolina General Statutes.

All current forms and requirements must be accessed and downloaded in the Magnus Health portal.

All High Point University undergraduate and graduate students are required to submit an immunization form in accordance with North Carolina General Statutes. High Point University utilizes Magnus Health as our health record affiliate to review and approve immunization requirements are met.

Magnus Health will send a welcome email to you HPU email address containing instructions to set up your account and submit the required information and forms for review. A team of nurses will review those documents and follow up by email if there are any item(s) that do not meet the established criteria, are missing or need additional information. The portal will display updates along the way, including when you have been cleared of all requirements. Reminder email messages containing specific details will be sent to your High Point University email address until all items are completed.

High Point University requires the following:

These items may be accessed, form downloaded, and all submitted in the Magnus Health portal.

  • Immunizations – form completed in its entirety with a doctor’s signature or stamp prior to submission.
    • Diphtheria Vaccination (DTap/Td) – 3 doses
    • Tetanus, Diphtheria & Pertussis Vaccine (Tdap) – Booster
    • Polio Vaccine – 4 doses
    • Measles, Mumps, and Rubella Vaccine (MMR) – 2 doses
    • Hepatitis B Vaccine – 3 doses
    • Varicella Vaccine – 1 does
    • Meningococcal ACWY Vaccine (MenACWY) – 2 doses
  • Tuberculosis (TB) Questionnaire – questions will be answered in the portal.

All items listed above must be approved by Magnus Health nurse review team in order to meet the requirements. 

Guidelines

  • A letter of religious exemption may be submitted in lieu of the immunization form and must contain the date and student’s printed name and signature. If the student is under 18 years of age, the letter must have the date and student’s and parent/guardian’s signature.
  • If you are medically exempt from receiving immunizations, we require signed and dated documentation from your primary care provider.
  • Be sure everything is completed in its entirety and signed before submitting in the Magnus Health portal. Multiple or partial submissions are not recommended unless directed by the Magnus Health team.
  • All required documents must be uploaded directly to the Magnus Health portal. No paper, email or faxed forms will be accepted by High Point University staff or the Student Health Center.
